With a focus on activities of daily living that influence health and result in a better quality of life, the doctors and other providers and staff at Stellis Health play an important role in keeping people healthy. We know that things like stable housing, transportation, food security, financial stability, and social networks are also essential for good health. This is where our care coordinators really shine. They work with patients and the entire health care team to set and achieve individualized health care goals along with finding ways to better support daily living activities.

Care coordination or chronic care management is yet another way that Stellis Health is helping our patients better manage their health. While not specific to patients on Medicare, chronic care management is a covered Medicare benefit and a type of service that Medicare really values. Younger adults or children with complex medical or social needs may also benefit from care coordination.
